I confirm that:

• I am using my own authorised account.
• Information I submit is accurate to the best of my knowledge, and I will correct material inaccuracies when identified.
• I will not knowingly submit false qualifications, impersonate another person, falsify reports or sign on another person’s behalf.
• I will upload information about other people only where authorised and necessary for the relevant task.
• I will protect my password, authentication codes and signing PIN, and report suspected misuse promptly.
• On a shared device, I will check the account shown before entering information or signing and sign out when finished.
• I understand that my electronic signature is linked to the particular document and choices displayed at signing.
• I understand that a signature saved on my profile is applied only when I confirm that particular document, and that replacing my saved signature does not change a document I have already signed.
• I understand that changes to signed records must be recorded through the authorised correction or revision process.

I understand that SHIELD records information and supports workflows. Its status indicators do not replace the company’s responsibility to verify evidence, assess risks or make required decisions.

Nothing in this declaration removes my statutory rights or excludes responsibilities or liability that cannot lawfully be excluded.
